What we do

Our inpatient units provide short-term rehabilitation for patients aged 18 and over, to regain independence, prevent unnecessary hospital admissions and support timely discharge from local hospitals.

People may be referred for rehabilitation following an acute worsening in their health (e.g., infection, illness, surgery, a fall, or stroke), or for adjustment of complex medication or palliative care. On admission to the service, each patient is assessed and agrees on an achievable and motivational care plan with the multidisciplinary team; advanced nurse practitioners, ward doctors and local GPs are on hand to manage medical needs.
